New Patient Registration

Catchment Area

Our Practice boundary is within the A31, A331, Lynchford Road and the Farnborough Road. We are currently open to new registrations if you live within the practice catchment area.

To Register:

Complete a patient registration form GMS1 and complete a new patient questionnaire (both available on the above “Registering Online” tab or from reception).

Please take the completed forms plus two forms of identification (passport/photo card driving licence and proof of home address in the UK from within the last 3 months e.g. utility bill) to reception.

Book an appointment for a new patient check (including blood pressure and urine check) with our healthcare assistant or practice nurse.

If you take regular medication then please make an appointment at the surgery, to see your doctor before a repeat prescription is due.

Please ensure that the information you provide is accurate and as comprehensive as possible.
